CSU Executive Order (EO) 1104: Reporting Suspected Fiscal Improprieties
University and auxiliary officers and managers have a fiduciary responsibility to act promptly when fiscal improprieties are suspected or identified. EO 1104 clarifies your roles and mandates the required procedures for campus reporting.
If you suspect or identify financial irregularities, you must promptly report them using the required campus procedures.

SAM 20080: Reporting Actual Losses or Errors of State Funds
SAM 20080 is a California State University (CSU) requirement for tracking and monitoring actual incidents of loss or error involving state funds to strengthen oversight and accountability. This requirement applies to all campus units handling state funds.
San José State University has implemented a centralized reporting and recordkeeping mechanism to ensure compliance.
Report Actual Losses and Errors
Effective immediately, you must document any incident of actual loss or error involving state funds based on the following thresholds:
- Losses from $950 up to $4,999.99: Document the incident using this form.
- Losses $5,000 and Greater: These are reported using a different process.
